This Policy outlines the terms regarding cancellation fees and procedures applicable to the Service's subscription options.

The Company offers two subscription options for the Service:

• A three-month plan, and
• A six-month plan.

Subscription fees range from $200 to $250 per month, varying by the duration and features of the selected plan.

Subscription fees include:

• A prescription for a CGM device,
• The CGM device itself, and
• Access to coaching and medical consultation services.

These offerings are provided in the initial phase of the subscription term and are integral components of the subscription fee.

3. Cancellation Fee

Due to the upfront costs associated with the initial provision of services and products, the Subscriber agrees to a cancellation fee if the subscription is terminated prior to the end of the agreed term.
